Theres this sicc ass trick called blocking. You can block communities, users and even posts! Even moreso, Lemmy is open-source. Crack open that IDE and write the functionality yourself if you want it.
I am actually with you kind of. Mastodon has a filter where you can block post with certain hashtags. Adding an optional metadata tag for NSFW post that users can selectively filter out would be a welcome feature in my eyes. I don’t want to see gore (and certain kinds of porn) in my feed at all in the first place.
