The distinct skeleton you can see in the foreground is presumably from a standing male (you can see what looks like an end-on angle of the shaft at the base) - most of the soft tissue is from presumably a female (from the overly distinct labia that’s visible) bent over in front of them.
So honestly, after staring at it for far too long (and having some coffee) I’m guessing this is just a manipulated image (but probably not outright AI generated, it gets the major anatomy right).
X-Rays do kinda look like two people doin' it already:
But it's really exaggerated here with the shape of the skull and blurring. The part that made me initially think Two People (besides seeing rad techs do shit like this entirely too often (don't do healthcare tech, it's the worst)) is here:
which really looks like a rear image of a male pelvis but that has shifted/was moving during imaging (image is for comparison, I couldn't find a matching example of a patient moving during an xray):
