I’m updating my Pixel Tablet to December 27 Lineage 21 update. Does the update only include the actual LinesgeOS install (the ZIP on their website) or does it include the other files? So I update the boot.img as well? If yes, should I just flash the one from this device’s page on Lineage site?

You can unzip it and see what images are in. There’s no need to manually update either unless you want to root which depending on which way you’ll need to only do it once, or if you have a custom recovery.
Unless you know what you are doing, please follow the documented instructions for your specific device.
Does the Updater in the settings app update the boot.img or just the LOS install?
boot.img is part of the “LOS install”.
