Mostly caused by the awful decisions and direction from people at the top. Maybe if they didn’t try to find as many ways to cut corners while actively trying to cover it up, they wouldn’t be in this position. If only the took the lesser profits and did it the right way they may not be in this position and people wouldn’t have lost their lives either.
