To receive a puppy, you must first submit Form HUD-9473126-K9B with an attached $200 money order. Processing time is 6-9 weeks upon receipt, and delivery is 2-3 weeks. The government is not responsible for the condition of how the puppy arrives. If your puppy gets damaged, additional forms and fees apply to request a replacement.
