Here is a continuously updated list of communities dedicated to health, or specific diagnoses, from all around the Lemmy network.
There are some disclaimers and notes below the list. Check them too.

How to use links?
For each community, there are two links provided - one is absolute, beginning with a ! and @ in the middle; the other is a Local_link. Try the local one first, it should open the community directly within your home instance, so you can join it, interact with it etc.
If the Local_Link doesn’t work, e.g. you get a 404 error, you are probably coming from an instance that isn’t “aware” of this community yet. In this case, right click or long-tap the link with the exclamation mark, select copy link (or copy url), and paste it into the search box of your home instance. After a few seconds to few minutes, it should show. You can open the link from there or open it from the Local_link from this list.

I’m not vouching for any of these communities, so check at your own discretion. A lot of them are abandoned or inactive, so if you see one without activity that might be interesting, consider bringing it back to life and crosspost here. Communities with no content or description whatsoever aren’t included.
Work in progress - if you know any other (or create a new one), comment and I’ll add it to the list. (Similarly, ping me if there’s a broken link, something that doesn’t belong, should be categorized differently or other feedback.)
Important: Some instances don’t federate with lemmy.world, most notably beehaw.org. To view communities on Beehaw, use the first link, not the local one. You won’t be able to post in these communities (or rather, beehaw users won’t see your post.) To interact, you need to have an account on Beehaw or some other instance that federates with them.
Interactivity with kbin instances and magazines may also be limited or delayed, although most of the time should work fine.
Important: Some instances may gravitate towards politics or philosophies different from your own. That’s the nature of the Fediverse, so keep that in mind. Inclusion of a health community of any instance does not mean endorsement of that instance.
