I was kinda figuring he lived in the Institute because being such a visible mutant he might not be able to get jobs elsewhere or be trusted. But I don’t really know X-Men lore.
I don’t know it that well either but iirc he’s not a medical doctor but has a doctorate in something else and also I think he does regular lectures at a uni as well. May depend on the continuity as well. Maybe someone more knowledgeable can correct me.

Hank McCoy has six PhDs, including at least a few in medicine related fields like biochemistry.
I don’t know if he ever picked up an official MD but he acts as the team’s doctor and is almost certainly the leading expert of mutant anatomy.
