Pedal cover gets wedged and stuck. Hard to dislodge, especially when driving at speed. Luckily, this owner had the wherewithal to figure it out.

Toyota had a similar issue with the accelerator pedal getting stuck. If I recall the cause, was people using multiple floor mats and those extra ones getting under the pedal when accelerating and panicking.
I had a Prius that had a recall for the factory floor mat doing that! Luckily I already had some nice hard plastic mats, but that’s a scary thought. RIP my beloved Prius, and fuck drunk drivers.
