I have lifelong trouble focusing and organizing , I get hyperfocused on some things but avoid other tasks, I sometimes speak out loud involuntarily, my mood shifts quickly depending on situations, I feel restless, sleep around 6 hours or wake sometimes, and my BMI is 15.
Could be Adhd, could be bipolar, could be malnourishment. What you need to do truthfully is touch grass, eat something, and let a doctor make the formal diagnosis.

Fucking eat calorie dense and nutritious foods until you are at a healthy weight. Then if you still have these issues the ADHD conversation can happen.

Protein is good but you need mass gainer. 30g protein, add in 100g carbohydrate like maltodextrin or find a mass gainer supplement with some vitamins and stuff. It’s not “good for you” and it’s not whole foods, but you are way underweight and liquid food can help.
We’re not talking about a deficit that can be made up with a couple 150 calorie protein shakes, you need 1000+ calories.
And no, low BMI isn’t a definitive symptom of ADHD. I know people who are obese and personally I was around 19 until I started to pay attention (hyperfocus) to diet.

ADHD refers to a spectrum of symptoms, it’s not necessarily a disease in itself. This is a very misunderstood concept, and I’m sure I will get pushback here on it because we get really used to our labels, but I’ve been through the system so many times and talked to a lot of doctors and have connected with the consensus. The DSM5, the manual from which we get all these terms and diagnosis talk, is always in a state of flux and is used as a guide for identifying symptom clusters and forming plans to ease harmful effects of those clusters, not labeling people’s identities or diseases, nor curing anything.
What’s all this have to do with your trouble focusing? That there is likely an underlying issue causing your ADHD symptoms, and this could be anything from an actual innate disorder like autism spectrum disorder, or it could be an effect of life experiences like PTSD from stress or abuse, or it could be your current lifestyle and living situation, it could be clinical anxiety/depression, or it could be something else entirely.
This is why the boring answer, which is most often the correct answer, is to consult an actual professional, not rely on asocial, sheltered, internet strangers because you will unconsciously seek out what you want to hear, not what you need to do to address the problem, which may in fact be something you really don’t want to hear.

There is the Executive Skills Questionnaire (ESQ-R), the Diagnostic Interview for ADHD in Adults (DIVA-5), tests for mood disorders, etc. that a professional could do to inch closer.
The speaking out loud, mood shifts, lack of sleep and low appetite sounds like mania, maybe pre-psychosis states, and any related disorders, like borderline, bipolar, or schizophrenia to me. If you’re in the middle east, it should be really easy to find a doctor to prescribe you some antipsychotics for it. In my experience, describing symptoms like that, they pretty much all are eager to prescribe them. And I am not a professional, the message is just “maybe, have a look at this” :)
Should antipsychotics be right for you, there’s also another upside with most of them: weight gain. The only time in my life I put on weight was on antipsychotics and it was quite dramatic.
And yeah, I don’t think you’re healthy enough for Ramadan. I imagine showing them Suras about who should not do Ramadan could shut them up.
