How about just ‘Secretary Trump’ or ‘Elons secretary’ it puts him in a lesser position and also plays on the fact that he’s a raging mysoginist.
I suggest changing it slightly to POTUSITO - President of the United States In Title Only.
Has the added bonus of -ito being a diminutive suffix in Spanish, and probably many related languages, so afaik one could read it as “little POTUS” 😁

Since it’s vaguely related to Hispanic food, it’ll upset him more.
He apparently really hated the “President Musk” comments.
I like Pino, I do think we need to add to it and get him from many angles. I can’t think of any that would piss him off enough. Something like Musk’s personal assistant so his name doesn’t even get mentioned.
